Intake & Admissions Coordinator

Uniondale, NY (On-Site)

Salary: $60,000–$65,000 + Performance Bonus
Schedule: Monday–Friday | 8:30 AM–5:00 PM

A rapidly growing home care organization on Long Island is seeking an Intake & Admissions Coordinator to join its team. This is an excellent opportunity for someone with healthcare admissions, intake, insurance verification, or patient coordination experience who enjoys working with people in a fast-paced environment.

This role is ideal for candidates looking to grow with an expanding organization while making a meaningful impact by helping patients and families navigate the home care admissions process.

Responsibilities

  • Manage the intake and admissions process from initial inquiry through enrollment.
  • Communicate with patients, families, referral sources, and healthcare professionals.
  • Follow up on referrals and maintain consistent communication to maximize admissions.
  • Verify documentation and coordinate admissions with internal teams.
  • Build and maintain relationships with referral partners.
  • Maintain accurate records and track admissions activity.
  • Deliver exceptional customer service throughout the intake process.

Qualifications

  • 2+ years of experience in healthcare admissions, intake coordination, insurance verification, patient access, or a similar healthcare support role.
  • Home care experience is a plus, but not required.
  • Experience with larger healthcare organizations (such as Optum or similar) is highly preferred.
  • Strong communication, organization, and follow-up skills.
  • Stable work history with demonstrated longevity.
  • Comfortable working on-site five days per week in Uniondale, NY.

Compensation & Benefits

  • $60,000–$65,000 base salary
  • Performance-based bonus opportunity
  • 10 days PTO (flexibility available)
