Case Manager - Job Description

We are seeking a dedicated and compassionate Case Manager to support homeless veterans and their families. In this role, you will be responsible for managing a caseload of 15 to 25 clients, providing comprehensive support to help them achieve stability and improve their quality of life. Your role will involve a variety of tasks from securing housing to assisting with obtaining essential documents such as driver's licenses and Social Security cards.

Key Responsibilities:

  • Client Support: Serve as a primary point of contact for clients, providing assistance in various areas including securing housing, obtaining essential documentation, and preventing homelessness.
  • Case Management: Conduct weekly check-ins with clients to monitor progress and address any emerging needs.
  • Appointments and Transportation: Accompany clients to appointments and housing checks as needed, utilizing company vehicles. Ensure that clients meet their scheduled appointments and maintain necessary follow-ups.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client interactions, progress, and any other relevant information.
  • Collaboration: Work closely with other service providers and community resources to coordinate care and support for clients.
  • Compliance: Ensure adherence to all organizational policies and procedures, including maintaining confidentiality and privacy of client information.

Requirements:

  • Driving Record: Must possess a clean driving record with less than 3 points.
  • Background Check: Ability to pass a federal background check, fingerprint test, and drug screening prior to starting.
  • Skills: Strong organizational and interpersonal skills with a demonstrated ability to manage multiple tasks and prioritize effectively.
  • Compassion: A genuine commitment to supporting homeless veterans and their families, with the ability to build trust and rapport.
