Job Summary: The Case Manager is responsible for assessing, planning, and coordinating services for clients to ensure their needs are met effectively. This role involves working collaboratively with clients, families, and multidisciplinary teams to develop personalized care plans and support clients in achieving their goals.
Key Responsibilities:

Client Assessment: Conduct thorough assessments of clients' needs, strengths, and challenges to develop appropriate care plans.

Care Planning: Collaborate with clients and their families to create individualized care plans that address their specific needs and goals.

Resource Coordination: Identify and connect clients with necessary resources and services, including medical, social, and community support.

Advocacy: Advocate for clients’ needs and rights, ensuring they receive appropriate services and support.

Monitoring and Evaluation: Regularly monitor clients’ progress, adjusting care plans as needed to reflect changes in their circumstances or goals.

Documentation: Maintain accurate and up-to-date case files, including assessments, care plans, and progress notes in compliance with organizational policies and regulations.

Collaboration: Work closely with healthcare providers, social workers, and other professionals to coordinate services and share relevant information.

Crisis Intervention: Provide support and intervention in crisis situations, ensuring clients receive immediate assistance as needed.
Qualifications:
- Bachelor’s degree in social work, psychology, nursing, or a related field; a master’s degree or relevant certification is preferred.
- Previous experience in case management, social services, or healthcare settings.
- Strong assessment, organizational, and communication skills.
- Ability to work collaboratively with diverse populations and multidisciplinary teams.
- Knowledge of community resources and services.
