The Case Manager/Resource Navigator is responsible for assessing, planning, coordinating, and managing services for clients to help them meet their healthcare, social service, or other needs. This role involves working closely with clients, their families, and other professionals to ensure clients receive the appropriate resources and support. Case Managers may work in a variety of fields, such as healthcare, mental health, social services, or rehabilitation, and play a key role in improving the overall well-being of clients.

Key Responsibilities:

  • Client Assessment: Conduct initial and ongoing assessments to determine client needs, challenges, and goals. Gather information through interviews, observations, and collaboration with other healthcare professionals.
  • Care Planning: Develop and implement individualized care plans based on client needs, preferences, and available resources. Ensure that plans are realistic and achievable, addressing both short-term and long-term goals.
  • Service Coordination: Coordinate services between healthcare providers, social service agencies, and other resources to ensure that clients have access to necessary treatments and supports. Facilitate communication between all parties involved in the client's care.
  • Client Advocacy: Serve as a liaison for clients, advocating on their behalf to ensure they receive the appropriate services and care. Provide emotional support and guidance, ensuring clients feel heard and understood.
  • Documentation: Maintain accurate, up-to-date case files and documentation for each client, including assessments, care plans, progress notes, and communication with service providers.
  • Monitoring & Evaluation: Regularly monitor clients' progress and reassess their needs. Adjust care plans as necessary based on changes in client health, resources, or personal goals.
  • Resource Identification & Referral: Identify relevant resources and community services available to clients, and make referrals as needed. Ensure clients are connected to the appropriate services to meet their physical, emotional, and social needs.
  • Collaboration: Work collaboratively with interdisciplinary teams, including healthcare providers, counselors, social workers, and family members, to ensure comprehensive care for clients.
  • Compliance & Regulations: Ensure that all case management activities comply with local, state, and federal regulations, as well as any relevant policies and ethical standards.

Qualifications: 

  • Bachelor’s degree in Social Work, Psychology, Nursing, or high school diploma.
  • Previous experience in case management or a related field is preferred.
  • Strong communication and interpersonal skills to build relationships with clients, families, and other professionals.
  • Excellent organizational and time-management skills.
  • Knowledge of community resources, services, and regulations.
  • Ability to handle sensitive information and maintain confidentiality.
  • Compassionate, empathetic, and solution-oriented approach to working with clients.
