Navigator – Supportive Housing Services
Location: Jacksonville, FL

We are seeking a compassionate and mission-driven Navigator to support individuals experiencing or at risk of homelessness through a supportive housing initiative. This role is ideal for someone with a strong background in healthcare or human services who is passionate about helping others navigate complex systems and regain stability.

Key Responsibilities:

  • Engage and build rapport with individuals in crisis

  • Assess eligibility for support programs and guide clients through the intake process

  • Assist clients in obtaining required documentation (IDs, income verification, etc.)

  • Provide transportation to Social Security, DMV, food stamp offices, and other essential services

  • Coordinate referrals and emergency support as needed

  • Maintain accurate and timely records in tracking systems

  • Work closely with Eligibility Specialists and Case Managers to ensure smooth transitions

  • Support clients in accessing housing, employment, and basic needs resources

Ideal Background:

  • 20+ years of experience in healthcare or human services

  • Direct support experience with individuals facing mental health or substance use challenges

  • Trained in crisis intervention, case management, client education, and community outreach

  • Experience working in psychiatry, behavioral health, or medical administration is a plus

  • Comfortable working in a fast-paced, client-focused environment

Requirements:

  • Associate’s degree in a related field or equivalent experience

  • Valid driver’s license, insured vehicle, and clean driving record

  • Must pass a Level II background check and federal drug screen

  • Ability to work flexible hours as needed to meet program and client needs
