The Case Manager provides direct support to individuals and families experiencing housing instability, focusing on those with prior military service. This role includes assessing client eligibility, developing individualized housing stability plans, conducting housing inspections, and connecting clients to essential services including employment support, financial management, healthcare, legal aid, and public benefits. The Case Manager collaborates with internal team members and external partners to ensure coordinated service delivery. Duties include maintaining accurate and timely client records in compliance with documentation standards, conducting home visits, crisis intervention, and coordinating emergency housing services. The role also involves participating in team meetings, trainings, and special projects as needed. Reports To: Case Management Team Lead Preferred Qualifications:
