Job Summary:

The Case Manager is responsible for providing comprehensive support and advocacy to clients, helping them access resources and services to improve their quality of life. This role involves assessing client needs, developing individualized plans, and coordinating with community resources to ensure effective service delivery.

Schedule: Monday-Friday 

Key Responsibilities:

  • Client Assessment:

    • Conduct thorough assessments of clients' needs, strengths, and challenges.
    • Develop individualized service plans based on client assessments and goals.
  • Service Coordination:

    • Connect clients with appropriate resources, services, and support networks.
    • Coordinate with other service providers, agencies, and community organizations to facilitate comprehensive care.
  • Advocacy and Support:

    • Advocate on behalf of clients to ensure they receive necessary services and support.
    • Provide emotional support and guidance to clients, helping them navigate challenges and make informed decisions.
  • Documentation and Reporting:

    • Maintain accurate and confidential case files, including case notes, service plans, and progress reports.
    • Prepare and submit reports as required by the organization, funding agencies, or regulatory bodies.
  • Crisis Intervention:

    • Provide immediate support and intervention in crisis situations, ensuring clients' safety and well-being.
    • Develop and implement crisis intervention plans as needed.
  • Program Development and Evaluation:

    • Contribute to the development and enhancement of program services based on client needs and feedback.
    • Participate in program evaluations and quality improvement initiatives.
  • Community Outreach:

    • Engage in community outreach activities to build partnerships and raise awareness about available services.
    • Represent the organization at community meetings, events, and other networking opportunities.
  • Compliance and Ethics:

    • Adhere to organizational policies, procedures, and ethical standards.
    • Ensure compliance with relevant laws and regulations governing client services.
