Key Responsibilities:

  1. Client Intake & Assessment:

    • Conduct thorough assessments of individuals' needs, including housing, mental health, substance abuse, employment, and other social services.
    • Develop personalized case plans based on client needs, goals, and available resources.
  2. Service Coordination:

    • Link clients to appropriate services such as temporary housing, mental health counseling, substance abuse treatment, employment programs, and medical care.
    • Collaborate with community partners (e.g., shelters, health providers, legal services) to ensure clients have access to necessary services.
  3. Advocacy:

    • Advocate on behalf of clients for necessary services, housing, and benefits.
    • Help clients navigate complex systems, such as public assistance programs or healthcare.
  4. Monitoring & Follow-up:

    • Track progress toward case plan goals and regularly follow up with clients to ensure continued access to services and support.
    • Provide ongoing case management, adjusting plans and interventions as needed to meet changing client needs.
  5. Crisis Intervention:

    • Provide immediate support in crisis situations, ensuring clients' safety and stability.
    • Help clients manage any emergent issues (e.g., eviction notices, legal issues, substance use relapses).
  6. Documentation & Reporting:

    • Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
    • Prepare reports for funding agencies, supervisors, and other stakeholders as needed.
