The Non-Profit Case Manager for the homeless population is responsible for providing supportive services to individuals and families experiencing homelessness. This role focuses on assessing clients’ needs, creating personalized care plans, and connecting clients to vital resources, including housing, healthcare, employment, and social services. The Case Manager works closely with clients to ensure their safety, well-being, and empowerment while facilitating their path toward stable housing and self-sufficiency.
Key Responsibilities:

Client Assessment & Support:
-
Conduct intake interviews and assessments to determine the needs of individuals and families experiencing homelessness.

Develop personalized case plans with specific goals aimed at achieving stable housing, employment, and long-term self-sufficiency.

Provide emotional and social support to clients by building trusting relationships and offering consistent guidance.

Resource Coordination:
-
Connect clients with community resources such as housing programs, mental health services, substance abuse counseling, job training, legal aid, and healthcare.

Assist clients in completing necessary documentation and applications for housing assistance, public benefits, and other support services.

Help clients navigate systems and organizations to ensure timely access to necessary resources and services.

Advocacy & Support:

Advocate on behalf of clients with external service providers, landlords, and government agencies to secure resources and housing opportunities.

Support clients in resolving legal, financial, and personal barriers to housing stability, such as evictions or criminal record issues.

Assist clients with life skills training, such as budgeting, job readiness, and self-advocacy.

Case Management & Documentation:

Maintain accurate and up-to-date case files and records, documenting client progress, goals, and services provided.

Regularly review and update case plans in collaboration with clients to reflect their evolving needs and progress.

Conduct regular follow-ups with clients to monitor their progress, ensure goals are being met, and make adjustments to the case plan as needed.

Collaboration & Teamwork:

Work closely with other case managers, social workers, and service providers within the organization and in the broader community to create a holistic support system for clients.

Participate in team meetings, case conferences, and training sessions to improve service delivery and enhance client outcomes.

Crisis Intervention & Conflict Resolution:

Provide crisis intervention and conflict resolution services for clients dealing with housing instability, health issues, or personal crises.

Work with clients to address immediate safety needs and develop strategies for managing future challenges.
Qualifications:

Education: Bachelor’s degree in social work, psychology, human services, or a related field (or equivalent experience) prefered not required.

Experience: At least 1-3 years of experience in case management, social services, or working with homeless populations.

Skills:
-
Strong interpersonal and communication skills, with the ability to work with diverse populations.

Knowledge of community resources, government programs, and services for homeless individuals.

Ability to work independently, manage a caseload, and prioritize tasks effectively.

Crisis intervention skills and the ability to remain calm under pressure.

Proficiency in case management software, Microsoft Office Suite, and data entry
