Job Title: Case Manager

Job Description:
We are seeking a compassionate and organized Case Manager to join our non-profit team. The Case Manager will work directly with clients to assess needs, develop service plans, and connect them to appropriate resources. This role involves advocacy, coordination with service providers, and ongoing support to help clients achieve stability and independence.
Key Responsibilities:

Conduct client assessments and develop individualized service plans

Coordinate and monitor services, referrals, and follow-up care

Maintain accurate case records and documentation

Advocate for clients and connect them with community resources

Collaborate with team members and partner organizations
Qualifications:

Bachelor’s degree in Social Work, Psychology, or related field

Strong interpersonal and communication skills

Experience working with vulnerable populations preferred

Ability to manage multiple cases and maintain confidentiality
