As a Case Manager/ Resource Navigator you will coordinates and oversee the care and services provided to clients, ensuring they receive appropriate support tailored to their individual needs. This role is prevalent across various sectors, including healthcare, social services, and mental health.

Key Responsibilities:

• Assessment and Planning: Evaluate clients' needs through interviews and assessments to develop personalized care plans.

• Coordination of Services: Arrange and monitor services such as medical care, housing, and social support to ensure comprehensive client care.

•  

  Advocacy : Act as an advocate for clients, ensuring they have access to necessary resources and services

• Monitoring and Evaluation: Regularly review and adjust care plans based on clients' progress and changing needs.

• Documentation: Maintain accurate records of client interactions, services provided, and progress reports.

   

Qualifications:

• Education: A bachelor's degree in social work, nursing, psychology, or a related field is typically preferred not required.

• Experience: Prior experience in case management or a related field is often preferred.

• Skills: Strong communication, organizational, and problem-solving skills are essential.

• Certifications: Depending on the industry, certifications such as Certified Case Manager (CCM) may be beneficial.

Schedule: Monday-Friday; 8am-5pm

Pay: 46-51k
