The Resource Navigator is responsible for assessing, planning, and coordinating services for clients to ensure they achieve their health, social, and personal goals. This role involves collaborating with clients, families, healthcare providers, and community organizations to ensure that each client receives comprehensive and timely care.
Key Responsibilities:

Client Assessment:
- Conduct initial assessments to determine clients' needs, challenges, and goals.
- Develop personalized care plans based on client needs, preferences, and available resources.

Case Coordination:
- Coordinate and facilitate access to services and resources for clients.
- Advocate for clients to ensure they receive appropriate care, services, and benefits.
- Maintain regular communication with clients, family members, and service providers to monitor progress.

Documentation and Reporting:
- Maintain accurate and up-to-date records of client interactions, assessments, and progress.
- Prepare case notes, reports, and other documentation as required by policy or regulatory standards.

Resource Management:
- Research and identify available resources, programs, and services that align with clients' needs.
- Assist clients in navigating complex systems and accessing support, including housing, healthcare, employment, and social services.

Care Plan Review and Adjustment:
- Regularly review and update care plans to reflect any changes in clients' needs or goals.
- Monitor clients' progress and ensure they are achieving outcomes as planned.

Collaboration and Teamwork:
- Work closely with healthcare professionals, social workers, therapists, and other team members to provide holistic care.
- Participate in multidisciplinary team meetings to discuss complex cases and share insights.

Crisis Intervention:
- Provide immediate support in crisis situations, including emotional support, crisis intervention, and referral to specialized services if necessary.

Client Education and Support:
- Educate clients and families about available services, rights, and responsibilities.
- Empower clients to advocate for themselves and make informed decisions.

Compliance and Quality Assurance:
- Ensure that all services are delivered in compliance with legal, ethical, and organizational standards.
- Participate in quality improvement initiatives to enhance service delivery.
Qualifications:

Education:
- High School education, a bachelor’s degree in social work, nursing, psychology, or a related field (may vary depending on the industry).
- A master's degree in social work (MSW) or other advanced degrees may be preferred for certain roles.
