About the Role:

We are seeking compassionate, mission-driven individuals to join our team as Case Managers supporting homeless and their families in Jacksonville, Florida. This is a direct-service position working closely with individuals facing housing insecurity. As a Case Manager, you will act as a guardian and advocate, guiding clients toward stability and self-sufficiency through individualized care and resource coordination.


Key Responsibilities:

  • Manage a caseload of 15–25 clients (victims and/or their families)

  • Conduct weekly check-ins with clients to assess progress and needs

  • Assist clients in securing and maintaining housing

  • Provide transportation to appointments using a company vehicle

  • Perform housing inspections and monitor living conditions

  • Develop and implement individualized service plans

  • Maintain detailed case notes and complete timely documentation

  • Collaborate with housing agencies, social workers, and support services

  • Close cases upon successful client stabilization or completion of goals


Qualifications:

  • Valid driver's license with a clean driving record (no points allowed)

  • Able to pass a federal background check, fingerprinting, and drug screening

  • Experience working with vulnerable populations, preferably homeless individuals or veterans

  • Strong organizational, communication, and problem-solving skills

  • Empathy, patience, and a strong desire to serve


Work Environment:

  • Position begins and ends at our office located at Jacksonville, FL 32204

  • Travel required within the community; company vehicle provided for appointments

  • Caseload may be temporarily high due to staffing growth but is expected to normalize
