About the Role:

We are seeking a mission-driven Community Outreach Specialist to serve as the initial point of contact for individuals experiencing homelessness—especially veterans and their families. Working from our downtown Jacksonville office, you’ll help clients navigate the path to eligibility for housing and support programs by guiding them through essential steps like securing identification, accessing benefits, and gathering documentation.

This role combines compassionate client interaction with hands-on assistance to ensure clients are ready to move forward with long-term case management.


Key Responsibilities:

  • Manage a caseload of 20–30 clients at a time

  • Receive client referrals from Intake Specialists

  • Assist clients with obtaining critical documents (e.g., state IDs, Social Security cards)

  • Help clients apply for public benefits (e.g., SNAP, Medicaid)

  • Provide transportation to appointments (DMV, Social Security, etc.) as needed

  • Maintain accurate documentation and track client progress

  • Conduct light field outreach as needed (e.g., visits to shelters or camps)

  • Transition eligible clients to Eligibility Specialists and Case Managers


Qualifications:

  • Some experience in social services, nonprofit work, or substance abuse support

  • Strong communication, organizational, and interpersonal skills

  • Empathy and the ability to work effectively with vulnerable populations

  • Valid driver’s license with clean driving record (or ability to drive company vehicle)

  • Must pass federal background check, drug screening, and fingerprinting


Work Environment:

  • Based at a Jacksonville office

  • Supportive team culture focused on helping people with dignity and urgency

  • Less fieldwork than expected due to high volume of inbound requests

  • Occasional outreach to camps or shelters may be required
