Position Overview: Under direct supervision, this position is responsible for receiving, coordinating, and distributing referrals for the Supportive Services for Veteran Families (SSVF) program. The role includes screening candidates for eligibility and working closely with other team members to ensure referrals are processed accurately and efficiently. The position reports to the Special Services Team Lead.
Position Responsibilities:
- Receive and respond to referrals for the SSVF program
- Screen referrals by phone, in person, or occasionally in the field, for eligibility, and assign referrals in coordination with the Director of Veteran Services
- Track all referrals and screenings and communicate outcomes to the referral sources
- Respond to client and community inquiries
- Complete referrals to other community agencies for clients who are ineligible for the program
- Maintain detailed spreadsheets related to client eligibility for reporting purposes
- Collaborate with the HUD/VASH program to ensure effective coordination of services, including providing training and guidance on the referral process and client eligibility verification
- Perform other duties as assigned
Position Requirements:
- Associate’s degree in a related field or equivalent work experience
- Minimum of 2 years of social services experience preferred
- Eligible to work in the U.S. and able to provide supporting documentation
- Must pass a Level II background check
- Must pass a federal drug screening; possession of a medical marijuana card is not acceptable under federal law
- Must have a clean 3-year driving history
- Must have a valid driver’s license, a registered vehicle without known issues, and the required vehicle insurance within 90 days of onboarding
